AND ENGINEERING (A / E) SERVICES AGREEMENT
BETWEEN

AND
CALVIN, GIORDANO & ASSOCIATES, INC.

NECESSARY FOR DESIGN OF BIKE LANES/ROUTES
IN AN AMOUNT NOT-TO-EXCEED $8,640 FOR THE
RIGHT-OF-WAY INFRASTRUCTURE IMPROVEMENTS PROGRAM
NEIGHBORHOOD N0.2 -NORTH SHORE
This Amendment No. 2 to the Professional Archite ral and Engineering (A/E) Services Agreement
(Agreement) made and entered this ~'"'~day of ~U I~f~ , 2007 by and between the CITY OF
MIAMI BEACH, a Municipal Corporation existing underthe laws of the State of Florida (hereinafter
referred to as City), having its principal offices at 1700 Convention Center Drive, Miami Beach,
Florida 33139, and CALVIN, GIORDANO & ASSOCIATES, INC., having its offices at 1800 Eller
Drive, Suite 600, Fort Lauderdale, Florida 33316 (hereinafter referred to as Consultant).
RECITALS:
WHEREAS, pursuant to Request for Qualifications (RFQ) No. 41-03/04-99/00, the Mayor
and City Commission adopted Resolution No. 2005-25976, on July 27, 2005, approving and
authorizing the Mayor and City Clerk to execute an Agreement with Consultant for Professional
Services for the North Shore Neighborhood Right of Way Improvement Project (the Project); and
WHEREAS, the Agreement provides for the planning, design, bidding and award,
construction administration and reimbursables for the Project; and
WHEREAS, the Agreement was executed for anot-to-exceed amount of $1,250,423; and
WHEREAS, on September 6, 2006 Amendment No. 1 to the Agreement was executed to
include the installation of "P3.2" classified water main replacements required by the City's Public
Works Department; for snot-to-exceed amount of $151,032; and
WHEREAS, this Amendment No. 2 to the Agreement is to include the installation of bike
lanes/routes required by the City's Public Works Department, in a not to exceed fee of $8,640; and
WHEREAS, this proposal has been evaluated by Hazen & Sawyer, the City's Program
Manager, and the CIP Office, and it has been established that this additional service request is fair
and reasonable; and
WHEREAS, this Amendment No. 2 increases the Agreement fee by $8,640, for a total fee of
$1,410,095.
NOW, THEREFORE, the parties hereto, and in consideration of the mutual promises,
covenants, agreements, terms, and conditions herein contained, and other good and valuable
consideration, the respect and adequacy are hereby acknowledged, do agree as follows:

ABOVE RECITALS
The above recitals are true and correct and are incorporated as a part of this Amendment
No. 2.
2. MODIFICATIONS
The Agreement is amended as defined in Schedule "A-2", attached herein.
3. OTHER PROVISIONS
All other provisions of the Agreement remain unchanged.
4. RATIFICATION
The City and Consultant ratify the terms of the Agreement, as per this Amendment No. 2.
IN WITNESS WHEREOF, the parties hereto have caused this Amendment No. 2 to be
executed in their names by their duly authorized officials as of the date first set forth above.

SCHEDULE "A- 2"
SCOPE OF SERVICES
Schedule "A" entitled, "Scope of Services", is amended as follows:
ADD the following under Task 5 -ADDITIONAL SERVICES, after the language added for
Amendment No.1:
Task 5 3 -Design of Bike lanes/routes:
The Consultant shall provide detailed design services for producing contract documents
(specifications and drawings) as needed for the construction of bike lanes/routes as required by
Public Works within the North Shore Neighborhood as noted below. In this effort, the Consultant
shall be bound by the requirements stipulated in TASK 2 -DESIGN SERVICES as adopted via
Resolution No. 2005-25976, on July 27, 2005, approving and authorizing the Mayor and City Clerk
to execute an Agreementwith CALVIN, GIORDANO & ASSOCIATES, INC for Professional Services
for the North Shore Neighborhood Right of Way Improvement Project.
In addition to revising existing design documents as necessary to accommodate new bicycle lanes,
the Consultant shall also revisit the encroachment tabulations, as new removals will result from this
effort. Demolition drawings shall be revised to indicate the removal of additional encroachments.
Landscape drawings shall illustrate revised sight triangle locations. In addition, bike lane and route
improvements shall be added to the 90% construction cost estimate.
In general, two 5 ft Bicycle lanes /routes will be provided on the north and south side of 73rd Street
from Dickens Avenue to Collins Avenue.
TIME OF COMPLETION
Additional design services contemplated by his Amendment are to be completed in within the base
Agreement schedule adopted via Resolution No. 2005-25976, on July 27, 2005, between the City
and the Consultant.
PAYMENT AND COMPENSATION
The method of calculating fees for the aforementioned proposed additional professional
architectural and engineering services is established in Exhibit `A-2' attached, based on
estimated labor plus direct costs. The total fee proposed for this Scope of Work shall be on a
not-to-exceed basis in the amount of $8,640. Invoicing will be monthly and based on hourly rates
as defined in the current Agreement between Calvin, Giordano & Associates, Inc and the City of
Miami Beach.
COMPENSATION
• Original Agreement amount $1,250,423.
Amendment No. 1 (Design services for installation of "P3.2" water mains) amount $151,032.
• Amendment No. 2 (Design services for the installation of bike lanes/routes) amount $8,640.
• Revised Agreement amount $1,410,095.
