POLICY MODIFICATIONS: This policy as issued is amended as follows:
In consideration of a premium of $47,955.00, it is agreed that coverage under this policy
will continue in force for a term of 36 months, commencing on the effective date of this
rider. This premium shall be payable in three equal annual installments of $15,985.00.
Each installment is due and payable on 1st of each year.

You means an individual who has given us Personal
Information in conjunction with:
a) asking about;
b) applying for; or
c) obtaining;
a financial product or service from us if the product or
service is used mainly for personal, family, or household
purposes.

As I had indicated in my email to you today, enclosed is the Renewal Rider for the Statutory
Accidental Death and Dismemberment Policy with Hartford Life. I had thought I had sent this to
the Clerk's office previously, but must have filed it with my contract copies here by accident.
I am still working on both the Humana and CompBenefits renewal contracts. As soon as they
are completed I will forward them both.
