(FRA) is a minority owned consulting engineering and surveying firm, which has been dedicated to providing professional services to government and private agencies since 1987. The FRA Team is comprised of nearly one hundred full-time professionals who take pride in doing their jobs exceptionally well. Relevant to this contract, FRA's knowledge and expertise lay in the area of Engineering, Surveying and Mapping. FRA's survey experience includes both prime and subconsultant roles on numerous roadway, drainage, design and construction contracts throughout South Florida for FDOT District 6, City of Miami, City of Ft. Lauderdale, Miami -Dade County, Miami Parking Authority, South Florida Water Management District. Qualified Team/Field Crews Our team consists of qualified multi -disciplined professionals with broad experience in all phases of surveying, mapping, utility locating and designating. We have 4 crews readily available at moment's notice to perform any required task. Knowledge of procedures, personnel and local conditions is a special strength of The FRA Team. The specific mission of the contract calls for the consultant, as directed by the City, to provide professional surveying, topographical and mapping services necessary including but not limited to preliminary engineering surveys; boundary/right-of-way surveys/legal descriptions and additional survey services. Prior to notice to proceed, FRA staff, will have a kick-off meeting with The City's Project Manager. The purpose is threefold: first, to discuss the overall project; second, to acquaint the CITY staff with The FRA Team members; and third, to define their expectations and the needs of the City. This contract will be Task Work Order driven. Detailed scopes for specific projects are provided when consultant services are required. FRA follows a schedule of events for each work order as shown in the Task Work Order Administration Approach shown on this page. Upon notification of a New Work Order, FRA will meet with the City, and schedule a field visit if necessary, to develop the scope. At that time, FRA would also discuss a project schedule with The City's Project Manager. Within one day, Mr. Gatell would submit a detailed scope and estimate. Upon approval and issuance of a work order, FRA would mobilize crews immediately. BASED ON THE ABUNDANCE OF OUR RESOURCES, OUR PREVIOUS EXPERIENCE AND GEOGRAPHIC REPRESENTATION, WE ANTICIPATE THAT WORK ORDERS' COMPLETION MAY BE REDUCED BY AS MUCH AS 25% ON THE AVERAGE. Prior to performing the work, we will customize a team of professionals, sharing our resources, for each work order. For each work order, an in- house kick-off meeting would be held to discuss objectives, methodology, procedures, timetable and deliverables. All research data in addition to applicable permits would be secured. At that point, field work commences. Field Crews are debriefed and fieldwork checked daily. Weekly progress and periodic safety meetings are held until the work order is completed. QA/QC takes place daily and prior to submittal. It is important to note that FRA is very particular about documenting, documenting, documenting! Field work is often videotaped and pictures are routinely taken. This eliminates future liability for the City. FRA has developed a documentation system which has proven beneficial to many City's throughout the State. It is depicted below in the flow chart. Preliminary Engineering Surveys The following is our methodology and approach to Preliminary Engineering Surveys 1. Establish geodetic baseline control on NAD 1983/1990 adjustment, using the Standards for FDOT Global Positioning System Survey for Horizontal Networks we will establish primary control points after discussing with the City Surveying and Mapping Consultant Management City and obtaining their approval to our G PS Plan and schedule. 2. After obtaining approval for the PNC primary control set up secondary control and using an electronic data collector with EFB software, make all necessary ties to Centerlines, Section, Subdivision, Lot, Block and Right of Way to re-establish all of the dedicated right of ways along the project as per the furnished Right of Way maps. 4ITHE R.ALEMAN TEAM M&A -2 m Citywide Surveying, Topographical & Mapping Services Methodology & Approach 3. Calculate centerline alignment and Existing, prepare an alignment geometry chain and a separate geometry chain for every R/W line calculated. With all this data we will prepare the Right of Way Control Map and submit it to the City for approval. Obtain approval in writing by the City Surveyor. 4. Stake and station the centerline alignment and any dedicated (by right of way map) connecting side roads at even thousand foot stations including the beginning and ending andof project stations and set references. 5. Establish permanent Bench Marks (poured in place concrete monuments and brass disc at one mile intervals, prepare a series of secondary Bench marks at 1000' intervals and outside of the limits of construction, using three wire method run through the bench marks to another established bench mark to obtain the adjusted elevation for the job. Provide the Consultant Project Management City a hard copy of the bench mark table including name, elevation, identify elevation datum, station and offset, and description of each Bench Mark. All data (including vertical datum and a copy of the Bench mark Table) will be recorded in the CAICE database and called "BM.rpt"). 6. Based on the City instructions we will either do a DTM of the length of the project or do only the Cross Section at 100' intervals and the topography. We will collect all this data with EFB and Process it in CAICE assigning all the features, etc to their corresponding levels and zones according to the City instructions. 7. We will follow the FDOT Design Survey Check -list for compliance with the State Guidelines, perform the necessary Underground Utility Surveys and follow the City guidelines, placing all the utilities in their appropriate zone and use the naming conventions of the designated utilities, we also verify the use of the appropriate attributes on the reports of the located test holes. 8. We will use all the City forms and directives to make sure that our final product will comply with the City's directives. Right of Way Survey 1. A horizontal control baseline should be established on N.A.D. 1983-1990 adjustment, following the FDOT standards for Global Positioning System Survey (G.P.S.) as amended on December 21, 2000. This horizontal control baseline is based on primary Horizontal Control/G.P.S. points and/or Secondary Horizontal Control points previously established by TH E CITY. At this point we prepared a Project Network Sheet, which is recorded in CAICE database, which we submitted to THE CITY. These sheets are signed and should be reviewed and approved by the City Surveyor Manager and/or City Location Surveyor Manager. and/or by the Designer Project 2. After the approval of the Project Network Sheet we reestablish the centerline or/and baseline of the existing right of way lines along the main corridor of the project, following the Right of Maps of the roads as provided by DOT. THE R.ALEMAN TEAM M&A -3 Citywide Surveying, Topographical & Mapping Services Methodology & Approach Early in this survey, we should obtain and study any original field notes and tie to above- mentioned centerline all Land Corners (Sections Corner and Quarter Corners). These land corners should be controlled by G.P.S. also. To reestablish said centerline Subdivision Maps, PRM(s) Block and Lot Corners and intersecting county road maps and/or city street maps will be very useful. A complete topographic survey should be performed after the centerline of the existing Right of Way line is under control; taking all field information which may impact or be impacted by the new design of the road or by the widening of the road. Among other items we will survey sidewalks, curbs, medians, edge of pavements, grass areas, manholes, valves, fences, poles (mastarm), trees, signs and adjacent buildings. At this time we will survey bridges, drainages, intersecting roads or ramps, etc. This survey will be done using a total station and an electronic data collector with THE CITY Electronic Field Book Software. 4. Aerial photography (raster imaging) may be a useful tool to verify the features already taking or any one missed. All information taken in the field is discussed and approved by the City Location Surveyor. Certified Right of Way Control Survey Maps After the Right of Way Survey is performed we proceed to prepare the Right of Way Control Survey Maps consisting in certified drawing of 24"x36" as well as the files will be transmitted on 3.5 inch diskette or CD's in Microstation D.G.N. format. These maps will show all alignment and all right of way survey data which may control the position of boundaries of real property, the drawings will be showing all details required for Right of Way Control Survey by the Minimum Technical Standards, Chapter 472FS and 61 617-F.A.C. and the Florida City of Transportation Procedures (Topic Nos. 550-030-012 and Topic Nos. 760-000 thru 760-012). The final set of Maps for Right of Way Control Survey contains: Key Maps, Detail sheets and Reference point sheet, Sheet 1 of the Key Maps has the following certification, I hereby certify this Right of Way Control Survey was made for the purpose of surveying, referencing, describing and mapping the centerline and/or baseline for the transportation facility shown and depicting hereon that said survey was done under my responsible charge and meets the Minimum Technical Standards set forth by the Florida Board of Professional Land Surveyor Chapter 21HH-6 Florida Administrative Code pursuant to Section 472.027 Florida Statutes. This drawing, consisting of sheets to is a true, accurate and complete depiction of a field survey performed under my direction and completed on (date). Certified and sealed by a Florida Professional Land Surveyor. Right of Way Map and Quality Assurance Review We prepare our projects in coordination with the roadway designer complying all his R/W requirements during the development of the different phases of the project. We will advise the design engineer for the necessity of the acquisition of Limited Access R/W, Controlled Access R/W, Perpetual Easements, Temporary Easements and/or License Agreements. Also when addressing R/W *THE R.ALEMAN TEAM M&A -4 Citywide Surveying, Topographical & Mapping Services Methodology & Approach requirements consultation with the City R/W Surveyor and the City Surveyor Manager is needed. For example when we consider that a R/W may be maintenance pursuant to section 95.361, of Florida Statutes or when the possibility of advance acquisition is immanent; these situations represent a considerable savings of money for the City of Transportation. Right of Way Projects will be prepared in accordance with the FDOT Right of Way Mapping Procedure 550-030-015; the presentation may be either line drawing or aerial photo base (raster imagining). Key Maps, Detail sheets, Ownership Sheet and Legal Descriptions will be submitted to the City Right of Way Surveyor for reviewing at the completion of the different phases of the project (30%, 60%, 90% and 100%) or any time if required. All stages of the preparation will be QA/QC and accompanying the City checklist for said stages of the project Right of Way Maps and legal descriptions will be electronic submittal. The files also will be transmitted on 3.5 -inch diskette or CD's in Microstation DGN format. Also a full size set of Right of Way prints will be provided. 1. Meeting with the Design Project Manager to be acquainted with the project and deadline as made for the final submittal than the different stages of the job. In the meeting to the project our marking plan in the office and software and hardware that we are going to use for the development of the project. At this P.M. should provide to us Scope of Service. 2. Our surveyor will visit the project site and review the R/W Contract Survey, base for the R/W Maps as well as other records necessary for the job as records plats, city or county R/W Maps for side streets and intersected roads, railroads maps, counts, etc. 3. With all the above information in our hands we prepare proposal for the manpower necessary for the job and our estimate of hours need for our team. 4. After the approval of our proposal we should receive the L.O.A. to proceed with the project 5. A kick-off meeting should take place at THE CITY with the project manager and the City Surveyor Manager or the City R/W Surveyor. In this meeting they should provide us with the Scope of Services if it was not provided in the first meeting. We will follow all the requirements of the Scope of Service and any special request. Also in this meeting we will confirm the date for the 30% submittal. 6. Delivery 30% Right of Way project, which include: a. Cover sheet (Sheet No. 1) containing all the information requested by the City R/W Surveyor b. Key Sheet (scale 1"=400' or 1"=200') as decided by the City R/W Surveyor. This sheet will contain a general information of the whole job, as Land Sections divide in quantities, baseline alignment, the limits of each detail sheet c. Detail Sheet (scale 1"=50' or 1"=100' as selected by the City R/W Surveyor). Showing: complete baseline alignment, all control points, curves and tangents. All land sections control points (found and set) describing each monument. All existing R/W lines, on the D.O.T. roads and intersecting streets or roads. Stationing the centerline intersection and the bearing of the intersection roads. All the adjacent plats, showing blocks, lots, streets and any corner movements found. *THE E2. ALEMAN TEAM M&A -5 m Citywide Surveying, Topographical & Mapping Services Methodology & Approach If any vacated streets in the area of the project should be shown with the recording data and/or city or county resolutions number for the release of the right of way. 7. Delivery — 60% Right of Way submittal of the Project: At the beginning of this phase we should be supply the engineering design file (electronically or print out also). This phase must revise any comments done by R/W City to the 30% submittal and it will include: The proposed R/W lines labeled as R/W lines The point tracts' property lines labeled as centerline, these lines are determined from the title search and supported by any evidence found in a field survey. AH parcels like Fee Simple Parcel, Temporary and Perpetual Easements, License Agreements, TIIFT Parcels with the Bubbles with the right assigned numbers. Title search should be marked 60% continuation with the appropriate parcel number The centerline(s) of construction tie to the baseline of survey. Most parcels are plotted from the centerline but tied to the Baseline of Survey. Adjacent building as indicated by FDOT R/W manual. Building impacted by the proposal R/W line should be a show with dimension necessary. At this 60% submittal an ownership sheet, showing property owner's names, parcel and parent tract remainders areas will be accompanied the set of maps. At the end of this plan all maps should be delivered to D.O.T appraisal City. 8. Delivery 90% Right of Way submittal of the project. This phase must revise any comments done by R/W City to the 60% submittal and it will include the final proposed R/W lines (labeled as R/W lines) of any change of the roadway design or any addition happens during the D.O.T review of the 60% submittal. They will transmitted in electronic Word processing and hard copies certified by a professional land Surveyor. 9. 100% Submittal of the Right of Way project. This phase must revise any comments done by the R/W City at the 90% phase at the same time a new review of comments done at 30% and 60% will be done. A final check list previously provide by the Right of Way City marking all issues addressing by US as consultants. The R/W maps in this submittal will contains the most update information related with plats, City or County Right of Way lines, streets or vacated, current names of the streets, any new city limits, or any issue requested by the P.M or the Right of Way City. Ground Penetrating Radar Ground Penetrating Radar (G PR) will assist in Damage Prevention of underground assets, the avoidance of underground hazards and guidance for excavation activities by identifying routes with the THE R.ALEMAN TEAM M&A -6 Citywide Surveying, Topographical & Mapping Services Methodology & Approach least amount of obstacles. FRA has provided underground location services in Miami -Dade County for Nearly 20 Years and routinely identifies utilities that may potentially impact or conflict with proposed design. Our team is intimately familiar with the federal highway administration's (FHWA) process to develop and accurately map buried infrastructure. FRA has expertise in the utilization of new & existing technologies which allow us to accurately identify, characterize and map underground utilities. GPR or 2d data uses 1 Channel GPR Data. The radar pulse reflects from the object which is identified in the Target. Subsurface Utility Engineering SUBSURFACE UTILITY LOCATING Example of Identified Targets: (2) 8 -Inch PVC W.M. Pipes FRA uses state-of-the-art vacuum excavation equipment to obtain the exact positions horizontally and vertically of existing utilities in conflict with proposed structures. The critical path of events for locating is described as follows: 1. FRA will coordinate with THE CITY to establish limits of investigation, the nature of the proposed work on site, utilities in possible conflict with proposed construction and other unusual situations which may relate to the project. FRA's crew will be mobilized in less than 48 hours for this project. 2. Prior to excavation, FRA will examine and obtain permits required to perform the requested services. 3. FRA will coordinate with all the concerned parties as required to insure efficient completion of the service and will adhere to all regulations and laws regarding notification of utility companies, including Florida Statute 556.101-111 and "Sunshine State One -Call". 4. A Maintenance of Traffic Plan will be developed upon evaluation of the site conditions in accordance with MUTCD and Index 600. The MOT plan will be submitted to the appropriate CITY office. The project schedule will also be coordinated with the City. 5. FRA will investigate all appropriate records which include drawings, plans, plats and maps obtained during the research phase of the project. 6. By using the plans supplied by TH E CITY, FRA will check any proposed structures that may be in conflict with existing utilities. 7. FRA will determine horizontal locations of buried utilities at the exact point of possible conflict with the City's proposed construction. A second scan of the proposed features will be conducted to search for utilities which may not appear on available records. 8. FRA will excavate test holes not to exceed 225 sq. in. THE R. ALEMAN TEAM M&A -7 m Citywide Surveying, Topographical & Mapping Services Methodology & Approach to expose the utility in order to investigate, evaluate, measure and record the following information: a. Horizontal and vertical location of top and/or bottom of utility referenced to project datum, tie all vertical control to NGVD 1929 datum using third order three -wire method; b. The elevation of existing grade over utility at test hole referenced to project datum; c.The outside diameter of utility and configuration of none -encased multi -conduit systems, compositions, when reasonably ascertainable; d. Paving thickness and type, where applicable; e. General soil type and site conditions; and f. Other pertinent information reasonably ascertainable from test hole site. 9. Backfill the test hole with the select material compacted in lifts to prevent later subsidence of the test hole. 10. Provide permanent restoration of the pavement within limits of the original cut. When test holes are excavated in areas other than roadway pavement, these disturbed areas shall be restored as reasonably possible to the condition before excavation. FRA will guarantee pavement restoration for 3 YEARS. 11. Furnish and install permanent markers directly above the centerline of the utility structure and in each excavated test hole and record the elevation of the above ground marker. 12. Record location of the survey marker and reference to project control and datum. 13. FRA will perform survey to the level of detail required by the City for this project. 14. Review and compare utility records with survey information and resolve any conflicts prior to proceeding with test hole data reports. 15. Produce location of vacuum excavation in TH E CITY CAD files by station and offset supplied by survey subconsultant. Convert any pertinent CAD files to a compatible format. 16. Submit final deliverables to the City. SUBSURFACE UTILITY DESIGNATING FRA's designating services will provide the City the most precise horizontal locations possible of existing buried utilities during the civil design phase of this project. This is intended to replace traditional records based utilities plotting and survey of the 'One Call' locates. The designators utilize a proven survey friendly marking system and prepare field drawings for surveyor's use. On this project all utilities will be located by FRA personnel. This allows us to present and certify information to +1.0', exceeding the requirements of the State. The critical path of events for designating is described as follows: 1. FRA will meet with appropriate TH E CITY personnel to establish project limits or areas of investigation, nature of the proposed work on site, utilities or structures of interest to the City and other special or unusual situations. THE R. ALEMAN TEAM M&A -8 Citywide Surveying, Topographical & Mapping Services Methodology & Approach 2. Coordinate with inspectors, property owners and others as required to insure efficient completion of the designating service. 3. Prior to excavation, FRA will examine and obtain permits required to perform the requested services. 4. Conduct a thorough review of records made available during the research portion of the project. These materials shall be used as an aid in the identification of the number, identity, size, and material or utilities and structures located in the field, and they shall not be used as a substitute for actual geophysical location. 5. A Maintenance of Traffic Plan will be developed upon evaluation of the site conditions in accordance with MUTCD and Index 600. The MOT plan will be submitted to the appropriate office. The project schedule will also be coordinated. FRA will use the most current geophysical prospecting equipment with various frequencies to detect a wide range of utilities. We will designate the horizontal location of all existing buried utilities as precisely and professionally as current utility locating including providing an approximate depth technique will allow at intervals determined by the City. APWA codes with paint, pin flags or laths will be used. 7. Our team personnel will perform an electronic sweep for unknown and undocumented utilities which may be within the project limits but not appear on the plans provided. 8. Produce field marking sketches reflecting the approximate location of all painted and flagged buried utilities. This will include any relevant field notes pertaining to the designating phase of the project. 9. FRA will survey utility designations to the level of detail required by the City. 10. Compile information using our automated systems and quality assurance procedures. Format and present field notes and comments to the surveyors for correction. This procedure is referred to as "final check" and ensures proper designating, survey and drafting of utilities. Survey, CAD & Documentation Our approach to the surveying task is the following. FRA will review the scope of services, secure maps, survey plats, aerial photographs, right-of-way maps and if necessary, original government field notes in preparation for the review of the site to be surveyed. After acquisition of documents, including review of the land description(s) either provided or obtained by THE CITY, the project surveyor will make an on-site inspection of the subject property to ascertain its individual requirements and problems. The project surveyor will then meet with the Project Engineer to formalize the procedures necessary for the project's timely completion, and an estimate is then prepared and given to the Engineer. We feel that at times it is advantageous to both parties to have the Project Surveyor meet in person with the THE CITY Surveyors to discuss their needs. Once a mutual understanding has been reached and a notice to proceed has been received by our firm, the work may begin. The administration would then schedule the necessary field crew(s) and equipment and deploy said crews on the project site within an agreed upon time after receiving notice to proceed. The party chief(s), licensed land THE R. ALEMAN TEAM M&A -9 m Citywide Surveying, Topographical & Mapping Services Methodology & Approach surveyor(s), will be briefed on the scope of services required and given the name and telephone number of the client in the event more information is required, or an unusual situation occurs. All survey work is accomplished in accordance with the State's Location Survey Manual (Topic No.500-030-100, and 030); FDOT Survey Safety Guide Safety for Location Surveyors (Topic No.550- 030-013); EFB User's Handbook; Interim Metric Guide Location Survey and R/W Mapping (Topic No.550-030-020); and Right of Way Mapping Handbook (Topic No.550-030-015); the survey work will meet the minimum technical standards for Land Surveyors as set forth in Florida Administrative Code Chapter 61G17-6, pursuant to Florida Statute 472.027, and any other special instructions. The survey data will be recorded on a SUBSURFACE UTILITY FORM and in a "CAiCE" Database. We will furnish the City with legible copies of all pertinent information utilized in making the survey, as well as the archived CAiCE database in a 3.5" diskette or other acceptable medium. 1. Contact THE CITY Project Manager to establish the survey requirements for the project and format for the final deliverable data and develop staff -hour estimates. 2. FRA will set horizontal and vertical control using the survey information provided by the City. 3. When applicable we will offset the baseline, on congested roadways when MOT is a concern. 4. Establish three BMs on the project site. These BMs will be set outside the limits of construction and will not exceed 300 meter intervals. Three wire leveling techniques will be utilized to begin the bench run at a known BM and tie to second known BM. FRA will evaluate their secondary control traverse points in the bench run for use in a subsequent data collection phase. Bench Mark Forms will be prepared as required. 5. On designation and test hole projects requiring EFB style, we will survey utility targets relative to project control. The survey data will be recorded electronically using an electronic field book and THE CITY compatible software. 6. We will collect the designated utility locations within the limits of the project. We will locate the Vvh positions established by our project team personnel. This will be included in the CAiCE database and all Vvh positions will be noted in the field utility chain sketches along with noting in the database graphical file. 7. Our survey personnel will provide all field notes, CAiCE database to include a text.dgn or cdg file, BM Forms and QA/QC Checklists. All QA/QC for this assignment will be performed in accordance with TH E CITY'S Survey and Mapping QA/QC Management Plan. This Management Plan will be utilized for all subsequent assignments as well. 8. Throughout the life of this project, the scheduling effort by FRA will allow for concurrent activities to occur whenever possible. The survey portion of each requested project will be performed in the most cost effective manner. FRA will take complete responsibility when performing survey work for this project. As the field work, and working drawings are finalized, a field verification will be made, and certified field notes and/or survey drawings will be produced as required by the client, and in the case of the Electronic Field Book, the Project will be reviewed, archived and submitted to the client. All drawings are reviewed, archived and submitted to the client. All drawings are reviewed by the project surveyor for correctness, completeness, and project accuracy. In the event only the field notes are required, the THE R. ALEMAN TEAM M&A -10 Citywide Surveying, Topographical & Mapping Services Methodology & Approach project surveyor will review the notes and certify to their correctness. Because we recognize the great value of the field notes as the record of the work performed, we will, whenever possible, utilize the note forms described in the City of Transportation "Location Manual". We feel these note forms provide the most straight forward approach for recording field data and leave little room for erroneous interpretation of the field notes. Deliverables The FRA test hole report furnished for every test hole is CAD produced in compatible formats using the TH E CITY's standards and reviewed four times for complete accuracy. FRA's in-house field sketching reports of utility markings provide the best complete information compiled during the excavation of the test holes plus any relevant survey information. A test hole summary is performed at the start and finish of each and every test hole project by FRA Team personnel to insure that each and every side test hole was dug at the requested location. Samples of drawings and reports are shown in the following pages. Quality Review Schedule and Quality Control The promotion of quality and our Corporate Commitment to Excellence is reflected throughout our organization and operations. For nearly two decades, FRA has been committed to providing quality work and responsible service to TH E CITY. FRA has developed a Quality Control Plan to be utilized for most any type of survey project we encounter and to provide an appropriate system of checking ourselves from the beginning to the completion of each project. As an end user of our own products, our goal is to provide a quality product that is reliable, complete, accurate, and in accordance with the contract scope of services. Key elements of our plan include but are not limited to defining the responsibilities of the QA Team, implementing a Four Level Quality Review Schedule and Equipment Maintenance. RESPONSIBILITIES OF THE OA TEAM One of the fundamentals of Quality Assurance is team work. We have a professionally staffed QA team with the expertise to ensure that your objectives relating to quality and consistency are accomplished. Reviews are conducted by staff with equal or higher qualifications as well as by independent peers. The Project Manager, Mr. Gatell, ensures continuing Corporate Commitment to quality assurance, ensures that the QA team meets the requirements of the City and provides Level Three Final QA review prior to submittal to The City's Project Manager. As a firm sensitive to your needs, it is FRA's practice that Mr. Gatell meet regularly with the City to evaluate any changes that might be required; pro -actively resolve issues that could affect our procedures; and ensure that we are exceeding high quality of standards expected on this project. The Project Manager also prepares fee estimates; manages work orders; maintains project correspondence and documentation; keeps meeting minutes; develops and maintains project schedules; designates the appropriate TRAINED project staffing for each element of work and ensures that applicable survey and utility standards are adhered to. THE ALEMAN TEAM M&A -1 1 Citywide Surveying, Topographical & Mapping Services Methodology & Approach The Project Manager conducts a kick off meeting with the project team where the following is discussed: safety, project goals and objectives, assignments and accountability, scope of services and procedures, the project schedule and critical deadlines, the budget, and project expectations, as well as other items that may need to be addressed. The Project Surveyor/Utility Coordinator, coordinates activities between staff and the FRA Project Manager, oversees day-to-day operation of surveying, mapping, locating and designating tasks and provides Level Two review. He assures that all supporting documents are signed and dated by the individual involved when the work is performed; that field notes are turned in daily; that the data collector is downloaded each afternoon or evening for daily review. The Utility Specialist/Sr. Party Chief, schedules work force crews, and performs work tasks, evaluates task methods and performs Level One field review of all collected data. He also performs equipment maintenance regularly. FRA'S SIGNATURE FOUR LEVEL QUALITY REVIEW TEAM APPROACH TO OUALITY ASSURANCE It is important to note that FRA believes that standardization and systematic repetition are key to assuring quality control and attention to detail. Our quality control process incorporates communication, checking, and back checking for the verification of changes to achieve a quality deliverable. FRA uses EFB Checklists, Quality Control Checklists, Designating/Locating QC Checklists, Survey Data Management QC Checklists, Project Evaluation Forms, Project Closeout Checklists, Agreement Task Checklists, and QC Tracking Stamp Requirements at different stages of the project. Samples of those checklists are shown below: Level One Review Field Review Conducted by FRA Sr. Party Chief/ Utility Specialist Survey Emphasis is made with the field personnel to maintain and follow all the standard procedures in the performance of the data gathering. We need to verify that the survey procedures follow the guidelines for the collection of electronic data and that the measurements in the field between known monuments check within the project tolerances. We require that all set up shoots at least 3 known control points. All traversing should be checked by the party chief and the field supervisor and all vertical loop closures data is verified and checked. Also make sure that regular shots have been taken at the back site during the data collecting, at least once every twenty minutes or so and at the end of each session. Utility The quality of the utility location data and the accuracy of the field work is checked daily in the field using a level one checklist. This review is the most critical in that most errors take place in the field. By completing the level one checklist, on-site, items that may not have been noted until late in the project can be resolved immediately while field crews are still in the field. Items such as the accuracy of horizontal and vertical measurements are insured; all measurements are duplicated verified and reviewed and checked for mathematical equality; vertical information must check to .01'; and measurement of the top and bottom of the utility structure will not be made until top and bottom are visually confirmed. THE R. ALEMAN TEAM M&A -12 Citywide Surveying, Topographical & Mapping Services Methodology & Approach Level Two Review Office Review Conducted by FRA Coordinator Survey All data collected is downloaded and processed daily. We perform check cross sections by conventional means as needed and at the intersections we verify elevations and location of at least 5 points to check our DTM's. As soon as the Topographical survey is plotted we send our project Surveyor or Field Supervisor to walk the Job and see if anything is missing or looks out of place before delivery of the DTM and Topo. After the plans have been approved we will commence to set the primary and secondary points to be used. If there is a need to deviate from the original plan we will contact the City's GPS Manager. After the primary points have been in place we will start with the secondary points GPS data collection. Each data collection session will be downloaded and processed in SKI PRO. It will be processed again in Hector the Vector. Once the process is complete, the results of the network will be reviewed and verify the constraints applied and the accuracy reports. After the City approves the network we will commence the traversing of the section and subdivision block points and field data collection. We will tract our segments and also the field crew that collected the information daily we try to processes the information as the project goes along. After the segment is processed we will check that results meet the City's requirements for the project. Once processed, the segment will be used for computing the basis for the Right of Way Control Map. This process will be maintained through the duration of the project, impromptu reviews will be performed to insure compliance with the City's Requirements. Our field segments will be submitted to the City 10 days before the end of the billing period, these submittals will be complete with copies of filed books, field book Q/A, Q/C checklist and along with media files, certification of virus, evidence of all of reviews for Q/A, Q/C prepared with each submittal. That will allow the City's Project managers to review the compliance with the projects development. All field segments are imported into the project CAiCE database and the project segments will be organized and logged, then the process of analysis starts. The project database will be used to compute and analyze. The positions of all the recovered monumentation found within the project. This is a process that goes through all section corners, block corners and compares their location with the theoretical positions according to the records available. As soon as the data for a section or a group of section has been gathered and analyzed an independent review by one of our senior surveyors of the computations will performed and we will start preparing our 60% Control Survey Map. This is an ongoing effort and might be reviewed as additional data is found and processed. If additional research is found to be needed this information will be added to give more strength to our determination and results. Utility The office review is designed to detect any items that the field review may have missed plus activities that are required to be accomplished within the office. Utility locations are cross-checked to be completely legible, reproducible and easy to understand. The FRA test hole report furnished for each hole is carefully reviewed for accuracy and consistency with field retrieved information. This level of review also gives a very good picture of the total project status. Upon acceptance of this level of review, the reports are generated and forwarded to the FRA Project Manager. THE R.ALEMAN TEAM M&A -13 m Citywide Surveying, Topographical & Mapping Services Methodology & Approach Level Three Review Project Management Review Conducted by FRA Project Manager Survey As the project advances, the following is checked: clarity format, level of detail and compliance with the City guidelines; Citys requirements; and color, level, line styles and text heights for Microstation/Geopack. This reviews will continue up to the 60% Map completion. After this 60% map is completed we will plot it and perform independent geometry checks based only on the information shown on the plotted maps. This then will be marked up and reviewed and will be added to our 60% submittal this process includes different checks and back check s to insure that all corrections are effectively placed on the map. After this process a final plot accompanied with the map checklists will be submitted to the City. This process will be the same for our Design Survey with only minor differences we will provide with the CD's for the virus scan certification required. This process will be repeated for our 90% submittal of the Control Maps, 30% Right of Way Maps and whatever submittals we do for the City. Each delivery will be checked thoroughly before submitting it to the City, reviewing all the files for using the latest DOT checklist of the graphic requirements. Utility The level three review ensures that all data and reporting required by the contract has been completed per TH E CITY procedures in a complete, accurate and timely manner. Reports that do not meet the FRA high standards for quality are returned for revision. By this stage, however, revisions are generally not needed. Upon final approval, the material is returned to the Project Manager for submittal to the City. Level Four Review Project Management Review Conducted by FRA PIC The Final review of our Q/A, Q/C Plan will be conducted by the PIC and an independent peer review. The peer review will include a review of the original scope of services; documented changes or amendments to the scope of services; all QA documentation; all THE CITY redline comments, if any; and a full redline review of the project mapping. This radar can accurately record the depth and location of pipes, cables ducts and various other underground utilities to verify and avoid design conflicts. We have been incorporating this service into our SUE services for several FDOT Districts, City's and County's Statewide. THIS PROCEDURE WILL SAVE TIME, EXPENSES AND WILL PROVIDE SAFE EXCAVATION OPERATIONS. • Our SU E and Survey personnel are cross -trained and can be used interchangeably - thus eliminating the need for separate Survey and SU E crews. • We will occasionally video or photograph a project to provide a visual check for the office review. Project Manager's Experience Summary Mr. Fernando Z. Gatell, PSM, is the Director of Surveying & Mapping for F.R. Aleman & Associates, Inc. and is responsible for all related operations. He has over 35 years of experience in the preparation and coordination of surveys which covers all of the facets of Surveying and Mapping. His expertise includes engineering, right-of-way, specific purpose, topographical, GPS, general land and aerial surveying services and elevation certificates. A brief summary of some of the Utility jobs he performs for other clients are discussed below: • 2001 - 2005 He provided all the utility coordination for the Town of Palm Beach, as a subconsultant. This project included 19.5 miles of water main improvements with in the Town.
