This Agreement, made and entered into this day of
by and between the STATE OF FLORIDA DEPARTMENT OF TRANSPORTATION, hereinafter called the "Department" and
CITY OF MIAMI BEACH
duly authorized to conduct business in the State of Florida, hereinafter called the Vendor."

WHEREAS, the Department and the Vendor heretofore on
Octobe08, 2005
entered into an agreement, hereinafter called the "Original Agreement" whereby the Department retained the Vendor to
furnish certain services in connection with Turf and Landscape Maintenance on Julia Tuttle Causeway (SR -112)
; and
WHEREAS, the Department has determined it necessary to amend the agreement to Chancle the Financial
Number from a phase 72 to a phase 78
NOW, THEREFORE, this Agreement witnesseth the following amendment le made;
The new Financial Number for the contract will be:25306617801
Except as hereby modified, amended, or changed, all of the terms and conditions of said agreement and any amendments
thereto will remain in full force and effect

Dear Mr. Oldenburg:
The Department's work program instruction for MOA's/JPA's establishes that the financial number
should use phase 78 instead of phase 72 as it is in the current contrail number. Therefore in order to
comply with the referred instructions an amendment to change the financial number to 25308617801
needs to be executed.
Enclosed are five (5) originals of the amendment to be executed by the City. Please return them to my
attention.
Please leave the date of execution on the top of the form blank so that it can be filled in by this office.
The Department will return an executed copy of this amendment for your records.
