SUBJECT: Fire Department Annual Emergency Response Statistics
Attached for your information is the Fire Department's Annual Emergency Response Statistics for the
past year and an accumulative report illustrating the past eleven (11) years, An analysis of the statistics
reveal since 1990 the total number of Fire and Emergency Medical calls have dropped from a high of
20, 278 in 1993 to a low of18,298 in 1999. The past year we experienced an increase of 339 total calls
over the 1999 low, Over the same eleven year period, we have gone from a high of 16,828 EMS calls
in 1993 to a low of 14,796 calls this past year, The average number oftotal calls during the past eleven
(11) years are 19,432, the average number of EMS calls are 15,883, This past year, total Fire and EMS
calls were 795 below average and EMS calls were 1,087 below average.
Since the high year for EMS calls in 1993, there has been a small decrease in the number of EMS calls
each year, except in 1995, which experienced a small increase, Since 1993, the average decline in the
number of EMS calls are 290,
Since the high year for total fire and EMS calls in 1993, there is also a small decrease, but with greater
fluctuation from year to year, There were increases in 1996, 1998, and 2000 from the previous years,
However, the average decrease since 1993 is 234 calls per year,
Though the trend indicates a reduction in the total number of calls since the highest year during the past
eleven (11) years, the average yearly reduction represents less than one (1) call per day, At this time, the
trends illustrated by the statistics are not significant enough to impact current staffing levels, deployment
strategies or response times.

Response to questions from City Management concerning Fire Department cumulative statistics over the
past eleven years.

There are several factors that may impact transport numbers. We can not control the number of calls we
get, but we can, and do manage the ones we get in the most appropriate manner. Some of the reasons we
feeltranspcrts are down include:
a. Total calls are down by nearly 800 compared to the eleven-year average. When ca11s are
down, there are less rescue responses, thus, less patients to transport.
b. Managed care has actively promoted programs to keep clients out of the hospital over the
last five years. Many call to authorize hospital care and are often directed to a doctor's
office or a walk.in clinic rather than approve a bospital visit or transport.
c. A demographic shift on Miami Beach from a large group of older retirees, who tended to
use emergency services to a greater degree, to younger. healthier people.
d. These younger healthier residents are more self.reliant, less likely to call 91 I, and if they
do have an emergency they will go to the hospital via private vehicle.
e. When transportation to a hospital is indicated, Rescue transports 93% of the time.
r. There is a trend in Medicine to keep provider costs down in capitated fee managed care
systems. This for us means fewer ER visits. Our statistics reflect the impact of this trend
on our system.

Yes. We are offering quality and timely Fire Rescue transports to more of our patients as well as dealing
with sicker (ALS vs. BLS) patients. Also financially, we are generating revenue for the city.

We should always seek to improve service, patient care, response times and public awareness. There should
not be a barrier to or fe.u of calling 911 for emergency help. People should not have to worry & o.:",t
transport fees before camng lor help. Provisions should be made in the City Ordinance not to char3-
residents, or to accept what insurance pays without aggressively bi\1ing patients. We receive few
complaints, but most of the ones we get revolve around transport fees. Public Service Announcements and
newspaper articles should be used to educate the public and explain the services we provide.

NO. While Rescue responses dropped 1.7"10 last year, we are seeing more ALS patients. The change is not
statistically significant to impact on staffing reductions.
Training requirements have also increased significantly over the past ten years. It is currently difficult to
fulfilllraining requirements without impacting coverage.
Changing demographics may reduce call108d in the short term, but with new construction and the vibrancy
of our city, the potential is there for massive incidents requiring multi-unit responses.
There are greater requirements for Incident Commanders on emergency scenes now than ten years ago.
Rapid Intervention Teams, Safety Officers and Passport Officers need to be assigned on every fire. This
requires us to utilize our resources more intensively on every fire or major incident.

Response time data is not reflected on the report.
Response times are impacted by street or lane closures due to on.going construction. ina-eased traffic
during daylight hours and nighttime traffic from entertainment district. The traffic light preemption system
should help improve response times. We don't have a benchmark or a way to measure response times
because of CAD inadequacies.

Cannot tell from this report.
The present deployment strategy is adequate south of 4 I St. An additiooal ALS Rescue unit in the north
end would ina-ease service to the North Beach NeighbOluoods. This unit has been requested in this budget
and the past two. This unit could be tried as a peek-staffing unit to determine long-term feasibility.
