Business- Business, Business Owner, or employee of a qualified business (must be 51%+ Hispanic owned) Nomination forms are available at City Hall and downloadable via the following link: http://miamibeachfl.gov/WorkArea/DownloadAsset.aspx?id=90688 (online form) and can be submitted via e-mail to leonorhernandez@miamibeachfl.gov or by mail to Leonor Hernandez, City of Miami Beach Building Department Administration, 1700 Convention Center Drive, Miami Beach, FL, 33139. In celebration of Hispanic Heritage Month, the City of Miami Beach honors those that have contributed positively to the Miami Beach community in their respective professional field and live or work in the City of Miami Beach. The nomination must include the nominee’s full name, address, phone number, and an explanation on how the person has contributed to the Miami Beach community. Detailed nominations must be received by Friday, August 25, 2017 at 5 p.m. The awards will be presented during the Miami Beach Hispanic Heritage Month Celebration on Friday, October 6 at 6 p.m. at North Shore Park. The Hispanic Affairs Committee serves as an advisory capacity to the City Commission and the City Administration in respect to matters pertaining to the Hispanic community of the City of Miami Beach.
