Aguila, Interim City Manager DATE: January 27, 2021 SUBJECT: UPDATE ON CITY OF MIAMI BEACH COVID-19 VACCINATION EFFORTS The purpose of this Letter to Commission is to provide the Mayor and Commission with an update regarding the coordinated actions the City of Miami Beach (“City”) has taken to administer the limited number of doses of COVID-19 vaccines received thus far. These vaccines are being distributed in accordance with federal and state actions and guidance, with an emphasis on facilitating the inoculation of eligible seniors in our senior affordable housing buildings and at the North Beach Senior Center (Unidad), as well as eligible homebound residents. The attached chart summarizes the cumulative number of doses delivered to the City, number of doses administered, and number of doses remaining (Exhibit “A”). SUMMARY OF CITY VACCINATION DATA (as of January 27, 2021) • Cumulative number of doses delivered to the City. The following doses have been delivered to the City to date: o First distribution of the Moderna mRNA vaccine – 600 doses o Second distribution of the Moderna mRNA vaccine – 500 doses o Third distribution of Moderna mRNA vaccine – 500 doses o Fourth distribution of Moderna mRNA vaccine – 600 doses1 o First distribution of Pfizer-BioNTech mRNA vaccine – 500 doses • Cumulative number of doses administered. The City has administered the first dose of the COVID-19 vaccine to 1,228 eligible individuals. Second doses will be administered beginning on February 3rd. • Cumulative number of eligible seniors transported to Mount Sinai Medical Center (“MSMC”) for vaccination. The Office of Housing & Community Services and Transportation & Mobility Department have supported MSMC’s vaccination efforts by providing free transportation for City residents with appointments to and from the inoculation site at MSMC. Due to supply constraints, MSMC has since canceled all first-dose appointments. Since vaccinations began, staff has registered and transported 235 eligible seniors to MSMC for vaccination between December 29th and January 8th. Free transportation will be provided for these individuals to receive their second doses at MSMC between January 26th and February 2nd, once they are eligible. 1 This distribution is allocated for second doses. DocuSign Envelope ID: 81F2F334-371E-47CE-8CD7-BD07E9131BD9 COVID-19 Vaccination Update January 27, 2021 Page 2 of 3 STATE AND COUNTY EMERGENCY GUIDANCE On December 23, 2020, Governor Ron DeSantis issued Executive Order (“EO”) 20-315 regarding vaccine administration. This EO directs all COVID-19 vaccine providers to vaccinate three identified populations: (a) long-term care facility residents and staff; (b) persons 65 years of age or older; and (c) healthcare personnel with direct contact with COVID-19 patients. Hospital providers in receipt of COVID-19 vaccines are authorized to administer vaccines to those individuals they deem to be at risk of exposure, which is why the City’s first responders have been offered doses in Phase 1 of the rollout. On January 21, 2021, the State Surgeon General issued a public health advisory, providing that every vaccine provider in Florida should ensure the recipient of the vaccine is either a Florida resident, or an individual present in Florida “for the purpose of providing health care services involving direct contact with patients.” On January 22, 2021, Miami-Dade County Mayor Daniella Levine Cava issued Emergency Order 32-20 which, in pertinent part, creates reporting requirements for vaccine providers (including the City), and provides that vaccine providers should not offer vaccination appointments unless enough vaccine doses have been received, and a reasonably accurate forecast can be made of the number of persons who can be vaccinated with such supply. LIMITED AVAILABILITY OF VACCINE DOSES The ability of the City and other vaccine providers to ramp up vaccinations has been greatly limited by supply constraints. Last week, for instance, MSMC suspended all first- dose appointments effective Saturday, January 23rd, including vaccination appointments coordinated by the City’s Housing & Community Services staff. (Note: individuals scheduled for second-dose appointments were not affected, and MSMC has advised that it would proactively communicate alternative vaccination options to those whose appointments were canceled.) POSSIBLE EXPANSION OF CITY’S VACCINATION PROGRAM The Administration has relied on existing personnel to coordinate the vaccination of eligible seniors and to schedule and transport eligible seniors to MSMC for vaccination. However, assuming additional doses become available, any expansion of the City’s vaccination program would require additional resources, including temporary staff. Additionally, any expanded effort would need to occur concurrently with coordination of second doses for individuals who were vaccinated by the City or transported to MSMC for vaccination. A significant fiscal impact would be associated with any expansion of the City’s direct vaccination efforts, currently estimated at $202,000 per month (including overtime and DocuSign Envelope ID: 81F2F334-371E-47CE-8CD7-BD07E9131BD9 COVID-19 Vaccination Update January 27, 2021 Page 3 of 3 additional staffing costs).2 We intend to apply for reimbursement from FEMA for all eligible expenses; however, the likelihood of reimbursement to the City is uncertain. CONCLUSION This availability and distribution of COVID-19 vaccines continues to be fluid and evolves every day. Staff is working diligently to serve the most vulnerable members of community and will continue to do so. The Administration has also continued outreach to hospitals and other vaccine providers to identify potential partnership opportunities. The biggest challenge, at this point, is the limited availability of COVID-19 vaccines. However, as supplies increase and as distribution accelerates, we anticipate that more vaccine doses will become available. 2 Staff’s monthly cost estimate, assuming the City administers first doses and second doses concurrently (i.e. on a parallel track), includes the following: approximately $156,000 in overtime costs for two Fire crews, $36,000 in temporary staff support, and $10,000 in miscellaneous expenses (freezers, supplies, etc.) This estimate does NOT include the support of management staff or departmental staff, which would be required to supervise the program.
