SUBJECT: Evaluation Committee Relative to Request for Proposals (RFP) No. 2022-320-WG,
for Vision Insurance Benefit Plan
The purpose of this L TC is to update the Mayor and City Commission on the status of Request
for Proposals (RFP) No. 2022-320-WG. The RFP requests proposals from qualified firms to
provide a vision insurance benefit plan to eligible employees, retirees, and their dependents.
The City received proposals pursuant to this RFP on May 9, 2022 . The Evaluation Committee will
review the responsive proposals in accordance with the criteria established in the RFP.
I have appointed the following individuals to serve on the Evaluation Committee :
• Marvin Adams, Benefits Manager, Human Resources Department;
• Krystal Dobbins, Financial Analyst Ill, Finance Department;
• Kenneth Ingersoll , Compensation Manager, Human Resources Department;
• Marcela Rubio, Assistant Director, Housing & Community Services; and
• Takisha Williams, Risk and Safety Manager, City of North Miami.
I have also appointed the following individuals as alternates :
• Tatiana Mendez, Sr. Human Resources Specialist , Human Resources Department; and
• Alberto Ventura, Assistant Director, Parking Department.
